First Appointment Registration

If you are registering with the Office of Disability Services for the first time, you must call our office or stop by to arrange a disability screening appointment.  You will not be able to receive your accommodations if we do not meet with you to review the policies and procedures of accessing accommodations (applies to housing AND academic accommodations).  To make an appointment or if you have any questions please call Disability Services at the New York City Counseling Center: (212) 346-1526. Please submit your disability documentation by mailing or faxing it prior to your appointment (see below for contact information).  

General Academic Accommodation Requests:

If you are a student in need of academic accommodations and you DO NOT already have documentation from your medical provider, your medical provider may submit a letter explaining your disability and your need for accommodations and/or submit the form below.  If you are request housing accommodations, your provider must complete the form directly above, as it pertains specifically to housing accommodations.  You will need to schedule a disability screening appointment by calling the Counseling Center to review your academic accommodation request(s). Please feel free to contact an ODS staff member at (212) 346-1526 if you have any questions.  

During the first appointment, you will need to complete an online demographic form (completed at the office) and you will review the disability screening forms during the appointment.  A copy of them are below.  If you wish to review and complete the forms ahead of the appointment and bring them to the appointment, you may do so.

Accommodation Letter Request & Update Forms

Students who are registered with ODS must request accommodation letters via the online forms.  If you are not registered with ODS, you cannot submit these forms.  You will first need to register with the office.  ODS strongly recommends submitting this form at the beginning of the semester.

Testing Accommodation Request Form (TARF)

Students must submit a TARF for EACH exam or quiz they wish to schedule with ODS. It is the student’s responsibility to return the form FULLY COMPLETED 7 days prior to the exam date and 10 days prior to the final exam date.

TARF Submission Options:

1.      Bringing the form to ODS, located within the Counseling Center @ 156 William St, 8th floor

2.      Scanning and emailing the form to the Accommodations Coordinator, Kate Sullivan, csullivan2@pace.edu or accommodatetesting@pace.edu

3.      Faxing the form to the Counseling Center: 212- 346-1530

Notetaker Forms:

If you are taking notes for a student with a disability, you must review and complete the Instructions for Note-takers and Contract Form at the beginning of the semester and return it to the Counseling Center Office Manager, 156 William St, 8th Floor.  Between the 8th-10th week of the semester, please submit the Payment Invoice (signed by the student for which you are taking notes) to the Office Manager so that your payment can be processed in a timely fashion.  

**Failure to disregard these steps may result in a delay of payment or not being paid at all.  YOU MUST ALSO INCLUDE YOUR FULL SOCIAL SECURITY NUMBER (VISA NUMBER IF YOU ARE AN INTERNATIONAL STUDENT).**


Disability Housing Requests:

Please complete the Housing Accommodation Request Packet if you are a returning student to Pace and will be living in residence--regardless of whether or not you have requested housing accommodations in the past.  You must submit disability documentation from a qualified medical provider.  The provider can submit a letter or the 'Professional Disability Assessment Questionnaire, which is included in the packet. Either type of documentation may be faxed (212-346-1530), emailed to the Assistant Director, or mailed to the office, along with the Housing Accommodation Request Packet.  Your request will be reviewed upon receipt. You must also schedule a disability screening appointment by calling the Counseling Center to review your housing accommodation request.  Please feel free to contact the Assistant Director of Disability Services at (212) 346-1526 if you have any questions.  For additional information, please refer to the First Appointment Registration section.  

**Please note that if you are a returning student, ODS is requesting that students submit the Housing Accommodation Request Packet 3 weeks before the week of housing selection to allow time for the Assistant Director to review disability documentation.**
