Federal Verification Statement of Educational Purpose Requirement

Federal Verification Identity and Statement of Educational Purpose Form (V4 and V5)

This form, along with a copy of your government-issued photo identification may be submitted in person at the Financial Aid desk in your home-campus Solution Center, or by mailing a notarized original with a notarized copy of your government-issued photo identification to your home-campus Financial Aid Office. The form must include a wet signature (name signed in pen). We are unable to disburse your financial aid until these requirements are received and processed.

How to submit your Identity and Statement of Educational Purpose Form

  • You must appear in person at your home-campus Financial Aid Office to verify your identity by presenting a valid, government-issued photo identification (ID), such as, but not limited to, a driver's license, other state-issued ID, or passport. The institution will maintain a copy of your photo ID that is annotated with the date it was received and the name of the official at the institution authorized to collect your ID. If in-person submission is required, you may incur travel or other related costs. You will sign the Identity and Statement of Educational Purpose Form, and the Financial Aid representative will collect the original. You may request a copy be made for your records.

  • If you are unable to appear in person to verify your identity, you must provide the information on page 2 of the Identity and Statement of Education Purpose Form (Option B: By Mail) and sign in the presence of a notary. The original notarized copy of this document must be mailed to the Financial Aid Office along with a copy of your government-issued photo identification. Faxes, scans, and emailed attachments of this document are not acceptable.
